Introduction
Since our dogs are only available to approved homes, we require the following questionnaire to be submitted. This helps us learn a bit more about you and your lifetstyle to help determine if one of our Labradors would be a good match for you. Your answers do not have to be perfect, just be honest with us so we can make the best decision possible when approving homes for our Labbies. Thank-You!

Do you currently own any other pets at this time?, If so please tell us what species/breed, age(s) and where do you acquire your pet(s) from?
What, "if any" past experience have you had with the Labrador Retriever breed in general?
Does everyone in your home AGREE to wanting a Labrador? How many people reside in your home? Children's Ages? Who will be the primary caregiver?
Do you own, rent or lease your home? Very important..if you rent or lease, do you have written approval for a dog over 60lbs? We may ask that you provide a copy of the pet agreement.
Where do you plan on keeping your pet: House, kennel, yard, farm, garage, basement, etc? Please be specific in telling us where your pet will be living.
What are your thoughts on spaying and/or neutering? Are you planning on breeding?
Are you interested in ANY type of dog sports such as Conformation, Hunting, Field, Agility, Flyball, Obedience? If so, let us know which "if any" of your past involvement in these areas.
Can you easily provide up to and some times exceeding $2000.00 per year for regular care, feeding, training classes, veterinary care and such associated with proper dog ownership?
Are you familiar with house training a dog or the use of a crate in the aid of housebreaking?
If you acquired a dog from us, and later in the future unforseen circumstances were to arise that would prevent you from keeping the dog, we REQUIRE that you offer us FIRST RIGHT OF REFUSAL. Do you understand what this means?
Do you: Work full time/part time, Attend School, Military, Retired, self-employed? Please list you business name or name of company you work for, school, etc and how many hours you will be away each day that the dog will be left home unattended.

Tell us about your lifestyle; Are you an active person, enjoy outdoors, or do you prefer to spend your time around the house?
Do you have any physical/mental disabilities or handicaps?
Have you ever purchased,adopted or have been given a dog and it did not work out? Please explain:
Are you willing to commit to a dog for the next 10-16 years? Do you see yourself as getting a dog and then if things do not work out...finding the dog a new home?
Are you familar with the American Kennel Club Written Breed Standard for the Labrador Retriever? Do you know the difference between American Kennel Clubs Limited Registration and Full Registration Privledges?
What color Labrador are you most interested in? Do you prefer a male or female? Any other specific's? If so, please elaborate.
When it comes to selecting a dog, what are the most important things aside from health that you would consider; Would they be the sex, color or temperament and in what order?
Do you feel comfortable with letting an experieced breeder select which puppy is best suited for your home based on the criteria in which you are looking for a puppy is met? If not, please explain your experience with puppy selection and evaluation.
